Are you a visionary leader in social work, passionate about driving excellence in patient care and empowering teams? Join our team as the Director of Social Work, Clinical Practice and Education where you will be a pivotal force in shaping our organization's social work services.

The Director Social Work, Clinical Practice and Education is an authority and resource for social work practice and education and provides leadership to enhance social work practice and care delivery across the organization through integration of practice, education, consultation, and coaching.  You will concentrate your efforts on assigned focuses/specialty areas to impact and influence practice development and performance support across the organization as well as achievement of strategic excellence goals.  You will coach and partner to empower autonomous service delivery at the nursing center level and influence customer experience, patient transitions, and clinical and operational outcomes. 

Position Highlights 
*Provide expert guidance and leadership in social work practice and education. 
*Develop and deliver comprehensive education and training programs. 
*Collaborate with interdisciplinary teams to optimize patient care. 
*Ensure compliance with regulatory standards and company policies. 
*Drive performance improvement through data analysis and targeted interventions. 
*Build and maintain strong relationships with internal and external stakeholders. 
*Create toolkits and onboarding processes for new social work staff and students. 
*Contribute to the development and implementation of standardized care delivery approaches.

*Master's degree in Social Work required preferred.
*Must possess any certifications and licensures as required by state of employment.
*Advanced professional certification such as Hospice & Palliative Care, Gerontology, Health Care, Case Management, Mental Health, Substance Abuse, and Clinical Social Work preferred.
*Minimum of five-year clinical experience in providing direct social work delivery to patients and their families in a health care setting and experience in providing consultation for at least three-years.
*Knowledge and experience in long term care required.
*Previous administrative and supervisory experience.
*Advanced skills in computer programs such as Word, Excel, PowerPoint, and Google Suite.
*Must be able to travel.
